Abstract

The rupture in the peritoneum is one of the most serious complications of the liver abscess

To study the epidemiological, clinical and therapeutical aspects of the peritonitis by rupture of the liver abscess.

It was about a retrospective study of patients dealt with the peritonitis by rupture of the liver of abscess from January lst, 2005 to December 31th 2011 in the teaching hospital Yalgado Ouédraogo of Ouagadougou in Burkina Faso.

Fifty one cases of peritonitis by rupture of the liver abscess were operated either 3.8 % of the whole generalized acute peritonitis .The average age of the patients was 38 years. There were 44 men (86.3 %) and 7 women (13.7 %). The average time of consultation was 9 days. The physical examination highlighted asthenic peritonitis in 34 cases (66.7 %) and sthenic in 17 cases (33.3 %). Retroviral serology was positive in 11 cases. The cytobacteriologic study of the peritoneum liquid was realized in 6 patients (11.8 % of the cases) and found 1 case of the pseudomonas. The medical treatment consisted of resuscitation and the administration of antibiotic associated with the métronidazole. The quantity of the aspired pus was on average 1 188 ml with extremes of 200 and 8000 ml. The liquid was chocolate in 35 cas (68,6 %).The seat of the rupture prevailed in the left lobe in 31 cases (60.8 %).An abdominal toilet and a drainage were systematic by tubular drains. Eleven patients presented complications (21.6 % of the cases).  Five deaths (9.8 %) were noted. The average duration of hospitalization was 14.2 days.

A sensitizing of the population on individual and collective hygiene, the misdeeds of self-medication should make possible the reduction of this pathology in our area.
