Les auteurs rapportent leur expérience du traitement des fractures ouvertes des membres chez 126 patients
hospitalisés dans le service d’Orthopédie Traumatologie du Centre Hospitalier Universitaire Yalgado
Ouédraogo, entre janvier 2003 et décembre 2004.
L’objectif était d’analyser les aspects épidémiologiques, cliniques et thérapeutiques de ces fractures afin
d’améliorer leur traitement. L’appréciation des résultats a été faite selon les aspects anatomiques post-thérapeutiques
et les complications. Ces patients relevant surtout du secteur informel, étaient âgés en moyenne
de 34 ans avec une prédominance masculine (sex-ratio de 3,7). Les accidents de la circulation (70,6 %)
ont constitué l’étiologie prédominante. Les fractures se sont localisées surtout aux diaphyses tibiale, fémorale,
radiale et ulnaire puis à l’épiphyse distale de l’humérus. Les fractures ouvertes de types I et II ont été
les plus fréquentes et 32,5 % sont situées au tiers distal de la jambe. Le traitement a été surtout orthopédique
avec seulement 20 ostéosynthèses secondaires. L’évolution a été marquée par 49 cals vicieux, 47
raideurs articulaires, 25 suppurations, 2 pseudarthroses, 2 thrombophlébites et un cas de décès. Nous
n’avons obtenu que 19,2 % de bons résultats. L’amélioration de notre plateau technique et la formation
d’un personnel qualifié permettraient une meilleure prise en charge de ces fractures.

The authors report their experience of 126 patients’ open limbs fractures treated at University Hospital
Center Yalgado Ouédraogo of Ouagadougou, between January 2003 and December 2004. The aim of this
study was to analyse clinical and epidemiological aspects of open limb fractures, in order to determine
which types of treatment realized and suggest appropriate measurements for better treatment.
This study included 126 open limb fractures of hospitalized patients. Nineteen patients were excluded.
Clinical and epidemiological parameters were analysed with epi info. 6. Results were tested according to
anatomical and evolutive aspects at the average follow up of 30 months.

There were 99 males and 27 females (sex-ratio 3,7) with an average age of 34 yeas. Road traffic accidents
were the most frequent aetiology with 70,6 per cent. The main open limbs fractures were localized on leg
in 69 cases, femoral shaft in 10, forearm in 10, foot in 7 and distal humeral in 6. According to Cauchoix
and Duparc classification, there were 34 types II and 25 types I and. Long segment’s open limb fractures
were localized in distal third in 55 cases, middle third in 17 and proximal third in 8 cases. Wound debridement
and setting fracture by plaster cast was the main method employed, with only 20 secondary internal
fixations. One patient was died. The main complications were constituted of 49 vicious callus,
47 stiffness of joints, 25 suppurations, 2 pseudarthrosis and 2 thrombophlebitis. Good results were obtained
only in 24 patients (19,2 %).
Road traffic accidents were the most frequent aetiology. Informal workers were the most exposed. Open
leg, femoral shaft, forearm, foot and distal humeral open fractures were the most frequent. Open leg fractures
were mostly localized in distal third (32,5 %). Setting fracture by plaster cast was the main method
employed, with only 20 secondary internal fixations.
Equipment of our operating theaters with appropriate orthopaedic materials and acquisition of qualified
personnel, could allow us to achieve a definitive treatment in limbs fractures.
