Double fardeau intra-individuel du surpoids et de l’anémie chez les femmes en âge de procréer au Burkina Faso

Contexte
Le double fardeau de malnutrition (DFM), défini comme la coexistence de la sous-nutrition et du surpoids chez un même individu, constitue un défi émergent en Afrique subsaharienne. Au Burkina Faso, où l'anémie affecte 28,0% des femmes et l'obésité 11,0%, peu d'études ont examiné les déterminants de leur co-occurrence. Cette étude vise à déterminer si ce DFM survient au hasard ou suit une tendance sociodémographique spécifique.
Méthodes
Nous avons analysé les données de 7987 femmes âgées de 15-49 ans issues de l'EDS-V du Burkina Faso. Le DFM a été défini comme la présence simultanée d'anémie (taux d’hémoglobine, Hb <12,0 g/dL) et de surpoids/obésité (Indice de masse corporelle, IMC ≥ 25,0 kg/m²). Les analyses ont utilisé des méthodes adaptées au plan d'échantillonnage complexe, incluant une régression logistique multivariée.
Résultats
Le DFM était présent chez 10,5% des femmes en âge de procréer. Comparées aux femmes du quintile de richesse le plus bas, celles du quintile le plus élévé (ORa=3,85 ; IC95% : 2,46 - 6,04) avaient une probabilité plus élevée de présenter le DFM. L'âge montrait une association positive avec le DFM (ORa=6,54 ; IC95% : 4,89 - 8,75) pour les femmes âgées de 35 ans et plus, comparées aux plus jeunes (<25 ans), tandis que le milieu rural était modestement protecteur (ORa=0,61 ; IC95% : 0,53 - 0,90).
Conclusion
Le DFM associant anémie et surpoids/obésité touche une femme en âge de procréer sur dix. Il varie selon certaines caractéristiques sociodémographiques, notamment le niveau de richesse, l’âge et le lieu de résidence. Ces résultats mettent en évidence des disparités populationnelles et soulignent la nécessité de recherches complémentaires pour mieux comprendre ses déterminants
